This session will center on care for women experiencing hypertension, diabetes, and thyroid disorders preconceptionally, throughout the pregnancy, and during the postpartum period. Participants will discuss pathophysiology, risk assessment, and management strategies, with an emphasis on a team approach to care. Transitions and handoffs among maternal fetal medicine and community based provider teams will be discussed.
